To be eligible, you must pass in all subjects of class 12 exam along with that your aggregate in Physics, Chemistry and Biology should be -

UR - 50%
OBC/SC/ST - 40%
PWD - 45%

If you fail in maths, you wont be eligible to give NEET UG Entrance Examination. Hence there is no question of admission in any medical college if you are not eligible to appear for NEET exam.
