Dear Venu, the validity of the exam fees for USMLE Step1 is limited to that particular exam only. It cannot be extended until subsequent attempts or it is not single fees until you pass. So you have to pay the fees separately every time you want to appear for USMLE 1. In other words, every time you register yourself for an exam, you need to make the payment.
