Dear aspirant,
No it is not.
The documents that one needs to carry on the exam day. Mentioned below are the names of the documents
NEET Admit Card
Aadhar Card is the most important document that one must present at the time of examination. The main purpose of is to support/ verify the details already present in the admit card.
Passport Size Photograph
Along with both the documents mentioned above, candidates are also required to carry multiple passport size photographs to NEET exam center. Photographs must be same as used in application form.

Hello aspirant,

There is no need to carry an application form in the examinatuon center. You only need yo carry admit card, valid photo id proof, a passport size photograph. One must be affixed on the admit card .
