Sports quota in MBBS is given on the state level only. First you give NEET, and then when you apply in different states then only you can apply for sports quota if your state does provide any reservation for the sports achievers. All the states in India Don't have the sports quota in MBBS. Few states that i know that have reservations for sports achievers in MBBS are Punjab, Andhra Pradesh and Tamil Nadu.  Hope this helps, thank you for asking question here at careers360.
