As you ask that .. is that compulsory to have name and date on photo  on jee main application form So..
According to the Examination Authorities..  name of candidate and the date ,when the photo was clicked..was to be on photo .... but if you missed to upload a photo with name or date ...so there is no need to worry... they  are not going to reject your application form .....so .... You will get your admit card for appearing in.jee main..
