Dear student,

The jee mains application form is released, but there is no such format given in the jee mains information bulletin for obc certificate or any other category certificate and neither it is told the valid date of issue for category certificate.

But as for the reference, you can check that for jee advanced 2020, the category certificate must be issued on or after April 2020. So you can get a basic idea from this that to which date you should make your obc category certificate. So, you should make a make an obc certificate and upload it which was issued after april 2020 or in this month ( just to remain on more safer side ).

Now as the proforma or format is also not available currently in information bulletin, then, generally I advise you to take the reference format from josaa 2020 counseling.
Now, in josaa 2020 there format of obc ncl was clearly mentioned. And generally this format remains same for jee mains exam.
