Undertaking form is to self-declare category status and complete the registration process accordingly.
Go to official website of NTA, download the undertaking form and fill it.
Once you fill the application form, get it scanned and then you can upload it.

According to the guideline provided by the official authority of JEE Main, you have to download and print the undertaking form. The undertaking form is available at the official site of JEE Main. After printing the undertaking form, you have to write your name and sign in that form.

You can upload the scanned copy of the filled and signed undertaking form in place of the document that you can not provide as for now.
