When ever you fill up an application form, its clearly mentioned that the name provided must be the same as present in any of the Government ID card. So you will face some problem undoubtedly. Now if you have the image in ID card through which it can be identified that its you, then you may be allowed or else you have to convince the person at the examination center to allow.
