Well, considering the level of competition in 2019, I guess a score of 423 would fetch your friend a rank near to 1 lakh. At this rank, a govt. medical college won't be possible and that's quite certain.
Now, comes the question of Private Medical College.
Well, it would have been a lot better if you could have mentioned about the Category too. Nevertheless, for UR it wouldn't have been possible at all,
But comparing the data with that of 2018, the reserved categories did have a shot to get a seat in the Private Colleges through Management Quota and the fees for such are generally quite high, reaching well over 40 lakhs.
You can apply through State Quota too to check out possibilities or go for a BAMS degree too.
