You can apply for PwD certificate online. Handicap certificate will be provided by the  Medical Education And Drug Department. Department will help you provide services in 10 days. First register yourself here:

Then , you can log in here after registering yourself and apply for the certificate:

https://aaplesarkar.mahaonline.gov.in/en/Login/Certificate_Documents?ServiceId=4092

Of if want to get it offline, then All India Institute of Physical and Medical Rehabilitation, Mumbai also issues the certificate after medical examination.
