Download the pdf of JoSAA Business Rules from the official site of JoSAA. Get a print out of the same. If you search through the pdf, you can find various annexures (including Annexure 7(b)) in the pdf. 

Keep atleast a couple of copies of the various Annexures, as they are very important during the JoSAA Counselling procedures. And fill them carefully.
