Your rank is too high to get you in a govt mbbs college as the minimum cutoff for this year is expected to be about 420 with a rank around 66000 in BC category however you may still apply for counselling to see if you might get a deemed university or BDS ( you may get a govt seat but even a  seat in a deemed university is equally good for BDS).
