Hello there!

In order to claim any reservation quota for admission, you need to produce the proof of the same. As this proof may differ from college to college and state to state, it would have been much better if you had mentioned the name of the college too, in your question.

As far as sports quota is concerned, a state or national level certificate is required. This too depends upon the college, as there are some colleges that does not accept state level certificates and only National level certificates are considered. Check the official website of the college of your choice to know the exact format of claiming reservation under sports quota.
