The important chapters of physics include dose of thermodynamics, moden physics, electromagnetism and mechanics varian numericals on different conceptual problems will be as day in and day out based on circuits as well as based on algebraic problems
In chemistry please paste special attention to the chapters of organic chemistry especially those of carbonyl compounds and other chapters of physical chemistry freshers atomic structure and covalent bonding and ionic balancing in inorganic chemistry since these are heavily questioned upon
keep giving MCQ examinations on a regular basis to help you with your question selection and time management skills

now that you have 5 months in hand before the examination you should work really hard to achieve your goal

chemistry important topics:

organic chemistry: carbonyl compounds, Alcohols phenols & Ethers, Chemical Bonding & then do the other topics . But practice a lot of questions for organic . Maximum questions in chem section are from organic .

physical chemistry- thermodynamics,equilibrium, electrochemistry, chemical kinetics & then go for other units 

inorganic chemistry- d&f block, co-ordination compounds 

study inorganic from Ncert only.

PHYSICS- I hope you have a good hold on MECHANICS that covers a major area for the exam & you get the lead among other students
