• NEET UG 2019 admit card
• NEET UG 2019 scorecard
• Pass certificate of Class 10 and 10+2
• Mark sheet of Class 10
• West Bengal MBBS 2018 provisional seat allotment letter
• Valid ID proof. (School or College ID card/Voter ID card/Aadhar card/Passport)
• Medical Certificate from Registered Medical Allopathic Practitioners
• Verification slip generated during pre-counselling phase.
• Final verification slip generated after the document verification.
• Category certificate
• Orthopaedic Physical Disability Certificate issued by a medical board of IPGMER Kolkata. Certificate issued by other authorities will not be accepted
