What should I fill in column [ state of eligibility (15% all India quota)] in NEET-2018 online application form..I belong to Telangana state

You have to fill the state name which you belong to so if you are from Telangana then it should be chosen there as from this year onwards Andhra Pradesh and Telangana also participating in 15% quota.
