Here is a list of various medical colleges in Delhi and their respective cut offs for admission under the State quota for the SC category in 2018.  This year the cut off is expected to be higher but you still seem to  some chances.
Under the All India quota you might have chances in colleges with lower cut offs like north eastern states, Tamil nadu etc.

if you want to go for the medical or dental courses then you have the choices of the college even through reservation you can get the govt. college if you apply through the state quota counselling. If you are delhi domicile then in delhi you have the options with your marks. If you cannot get the seat then you can also go for the physiotherapy, ayurvedic, nursing, dental and also  for the Bsc courses. 495 marks are not bad as the marks are quite low for the govt. seats but you can try for the other courses also. You can try for the delhi govt. college for dental course as the cutoff for all india quota for delhi is 431 with the rank 55442 in SC category.
