Hello,
Last date for correction of particulars in jee mains application form is now over on 22nd January 2018.So,You can't correct it now.However you can still fill other engineering entrance exam forms like VITEEE,SRMJEEE,BITSAT,UPSEE,COMEDK,WBJEE,...and many more.

No problem in that because photo verification is only compulsory at examination centre of jee mains.If there isn't any name or date written then you will not face Amy problem.Remember your photograph needs to be clear.
