Hello,
Fill the marks where you have secured highest. If at any place in the form asks for any improvement then you can write the details there.
Fill the updated mark and if it asks for yes or no or appeared for improvement exam write the appropriate answer.
You have to fill the application accordingly. So before filling the form read it carefully for the guidelines. After that fill the form so that you don't make any mistake
