For first 15 days, revise all the concepts of PCM, 5 days each. Plan for these 15 days so you can revise every topic. For next 15 days solve question papers. After solving each question paper, understand the solution of the question in which you made mistakes. If needed, recall the concepts. You should increase study time to minimum 12 hours. Stay away from things that distract you. Hope it may help you.

Is the board weightage for JEE MAINS 2017 is for combined top 5 subjects or for PCM also its 75% ?

As per new  eligibility criteria  from this session, Candidate should have secured at least 75% marks in the 12th class examination, or be in the top 20 percentile in the 12th  class examination conducted by the respective Boards for NIT/IIT/IIIT admission. For SC/ST candidates the qualifying marks would be 65% in the  12th class examination.

It is mandatory for aspiring candidates to have taken 5 subjects in 12th or qualifying exam. The 5 subjects for B.Tech/B.E. are:
1-Language
2-Physics
3-Mathematics
4-Any one of (Chemistry, Biology, Biotechnology, Technical Vocational Subject)
5-Any other subject

Assuming type of question as exam pattern you are asking for, so details are given below -

Number of questions - 180
Types of Questions - Multiple Choice Questions
Language - English, Hindi, Assamese, Bengali, Gujarati, Marathi, Tamil,Telugu, Oriya and Kannada
Marking Scheme -  4 for each correct response and -1 for each incorrect answer
Types of questions: NEET 2017 question paper will have objective type questions only, which means, each questions will consist of 4 responses, wherein, candidates will choose the most appropriate answer from the given options. Questions will be based on NEET syllabus as prescribed by CBSE, the exam conducting authority.

You don't need to be get tensed. If you want to crack this exam you need to be self motivated and determined. You can first focus on the chapters which have more weight-age in the exam.
