So, basically you are asking can you get admission in another college directly into second year, honestly speaking khushi not all colleges allow direct admission into the second year. It depends on the college  rules. Some colleges allow it if the syllabus and subjects are same in both colleges and if seats are available. So,I think you should send a mail to colleges in which you are interested to take admission and ask them if they accept students in second year.

Along with that you should have some important documents like your first year marksheet ,No objection certificate from your college and a migration certificate.

So,yes there is a possibility of you getting admission in private colleges.

According to your scores you may be eligible for central University of South Bihar , Babasaheb Bhimrao Ambedkar University Lucknow, University of Jharkhand,Central University of Karnataka, Central University of Haryana,Mahatama Gandhi Central University.You have high chances of getting in above all these that I have mentioned.

Getting admission in top universities according to your scores is difficult but still you can try to attend couselling maybe you get admission in later or spot rounds.

Yes, there are a few INC (Indian Nursing Council) approved B.Sc Nursing colleges in Delhi where direct admission without entrance exam may be possible, especially under management quota, NRI quota, or for vacant seats after counseling rounds. Here's a list of such colleges:-
