Hello!
Yes, you should apply with a new form. You might have to register all over again. Else your registration can be cancelled.
You can avoid doing it if the mistake wasn't a big one. But if it was a major one like filling incorrect marks, your personal details or any ug details, then you should definitely start from a fresh form.
