1. Class 10th Marksheet & Passing Certificate
2. Class 12th Marksheet & Passing Certificate
3. Category Certificate
4. Sub-Category Certificate
5. UPSEE Admit Card
6. UPSEE Rank Card
7. Domicile Certificate
8. Domicile Certificate of Parents (If pass the qualifying exam outside of UP)
9. Character Certificate
10. Medical Certificate
