You haven't mentioned your cut off. Neverthless, in 2018, the cut offs for various categories under State Quota are as follows:

General Category: 473
SC:  372
ST: 230
SE: 120

So at 454 marks if you belong to General Category, it os difficult for a seat in a Government Medical College. But for Reserved Categories you have a decent chance at any Government Medica College. Having said that, cut offs change every year and are dependant on various factors like no.of seats available, no.of candidates appearing for the exam, top NEET score and difficulty of the exam. Neet cutoff for government medical colleges in gujrat ranges from 620 to 470. I'm sorry but it might be quite difficult to find a government college with that score.

Still you should apply for the counseling process in order to check if you are eligible for any government colleges. According to my knowledge you might be eligible for admission in some other states so you should apply for the all India quota counselling also.
