Yes it might create a issue later so we recommend you to contact the cet administration people directly and resolve the issue as any such issue mught led to disqualification of your application as a whole.
After contacting them and they'll guide you better on how to rename/ change the personal details in the application form moreover in case of any issue later on you can say that you had already got the issue solved by the administration itself.
