Earlier till 2017 there are separate examinations for different states and universities but from now onwards neet is mandatory for all medical courses save pharmacy courses.

To fetch the seat in any college you must be neet qualified and it depends on different states some states has less cut off marks than others. Marks around 400+ are safe to fetch government college and I think counselling will start from August after mbbs counselling

In both All India and state counseling they have reserved seats for mbbs, bds, bhms and bams. But state has more seats so you should give state a higher preference over All India. States have more seats for bams and bhms.
