As per the information Brochure of NBE FMGE,  candidate has to submit the following self attested scanned documents to NBE-
Copy of MBBS pass
Copy of MBBS pass certified by the embassy
Copy of Adress proof
Two photographs
Copy of DOB proof
Copy of passport of the applicant
Copy of 10+2 passing certificate
Copy of 10+2 Marksheet
Copy of internship certificate (if done abroad)
Copy of eligibility certificate issued by MCI (if applicable)
