Admission to government medical colleges is done under two categories. All India quota (15%) and state quota(85%). Each state declares a seprate merit list for candidates of that state.

With a AIR of 47000 and OBC rank of 19000 it will be nearly impossible to fetch a college in All India quota. However, you have high chances of getting lower ranked government colleges in your state. So, search for colleges in your state and follow your state's counselling process for admission.
