No. The application form is not meant for MBBS admission. It is meant for undergraduate degree courses which includes B.sc Nursing, Allied health sciences degree and Diploma or fellowship or M.Sc, M.Ph degree courses for Nursing and allied health science diplomas, Post graduate diplomas, M.Sc and M.PH (Master of Public Health).

You will only be able to apply for CMC once you get your NEET Score. All the best. Keep working hard. :)
