Don't think that these are poor records. You have very decent academic records. Now, focus to gain good percentile in CAT for good college. Only few percentage weightage for academics records. You have 5+ year work experience that make you more eligible than other candidates who don't have work experience. So focus on CAT. Try to achieve 95+ percentile. You can get good IIT or Top Private Colleges.

The 5 year work experience is a very good opportunity for you . 

Your scores are average not poor .

If you work hard and prepare well for the entrance exams , You can easily get a good B School . There is a specific Executive MBA course for the people with good work experience . You should apply for that course . As an advise I would suggest you to do try in ISB , Hyderabad.
