To apply for B.Pharm you will have to buy prospectus with application at 500/- by sending cash or DD to the Principal, Elims college of Pharmacy, Thrissur. Fill the application form carefully and send it along with the certificates below to Principal, Elims College of Pharmacy, Villadam, Ramavarmapuram P.O, Thrissur – 680 631, Kerala, India

1) Certificate that proves your age

2) marklist of qualifying examination

3) TC and Conduct certificate

4) Migration certificate

5) if reservation applicable caste certificate (SC/ST /OBC candidates)

6) Income certificate

7) Medical fitness certificate

8) Photographs ( 5- passport size, 3-stamp size)
