Fill the application form carefully and send it along with the certificates below to Principal, Elims College of Pharmacy, Villadam, Ramavarmapuram P.O, Thrissur – 680 631, Kerala, India

1) Certificate that proves your age

2) marklist of qualifying examination

3) TC and Conduct certificate

4) Migration certificate

5) if reservation applicable caste certificate (SC/ST /OBC candidates)

6) Income certificate

7) Medical fitness certificate

8) Photographs ( 5- passport size, 3-stamp size)

You have not specified the college for which you seek admission for. However the admission procedure usually starts after the declaration of class 12 board exams results. Some universities also organise entrance exam for the same in the month of May.
