I suppose you are talking about Admission forms of Jadavpur University, Kolkata as i can see it from the tags you have used. You can fill up the application form for MSc starting from 11 April 2019 upto 26th April 2019 after which the entrance exam Will be conducted from 9th May 2019 upto 13th May 2019 proceeding with the announcement of results on 30th as well as 31st May 2019 afterwards the admissions will be made starting with 8th june 2019 uptil 14 june 2019.

Eligibility : You have to get through the Entrance Test and the Qualifying Exam 

Filling the form : At first you have to fill up the required details and you can now select the department you want to get enrolled to and you can select as many no. of departments you want to, you will get different application no. for each department you apply to afterwards proceeding with the fee payment.
