• Marksheet and Degree Certificate of Qualifying BDS Examination

• Certificate of Internship Completion

• Provisional or Permanent Certificate from DCI (Dental Council of India) or SCI (State Council of India)

• Class 10th Certificate (As a proof of Date of Birth)
