TISS accepts state NC OBC certificate. It accepts the certificate issued by revenue department of the particular states. Your parents income certificate should be enclosed with it. There is a time limit for the certificates issued. Make sure you aware of this.

A sample of prescribed format is given by TISS and you have to fill the details accordingly. They don't accept certificate of any other format.
