As you have mentioned that your neet score is 141 marks and your rank is 45324 which indeed is very high. But you haven't mentioned your category. Nevertheless, your marks are so less that surely you won't get any government medical colleges for any course.. But if we look over cutoff for MBBS in private colleges then you might have scored at least 430+ marks... This would differ somewhat for different categories. But with 141 marks you can get a private college for BDS course...

Don't loose hope.. You will get a private college....

I am sorry to tell you but 141 marks in neet is extremely low,  with this much marks you will not get a seat,  neither in mbbs nor in bds , actually with your marks you are not eligible for courses like bhms , bams .
I suggest you to prepare again for neet and clear it next year with flying colours and fulfil your dream of becoming a doctor.

Hello student as per source you can't get admission in government / private medical college as we all know that seats are very limited and admission is directly based on category rank and your category rank is not good.
