According to the level of the paper this year the  competetion is very high and the cutoff may reach upto 600 this year and bihar is one of the most competetive state.

But it all depends on your category as for Gen/Obc to get a MBBS /BDS seat this year at this score is very difficult in any of the states via the all india quota.

I believe at a score of 524, if not Government you have slight chances in securing a seat in any of the Bihar State Medical Colleges under Home State Quota. Do keep a check on the state counselling procedure and dates.
