As per KVPY eligibility criteria, candidates studying in class 11, 12 and UG first year are eligible to apply for SA, SX and SB streams, respectively. The eligibility is different for each stream SA/SX/SB. You can check the complete information about KVPY with the given below link.

Yes students can in fact apply through all 3 channels of admissions that is KVPY Exam, Jee Advanced and SCB Test. 

However one must satisfy eligible criteria for all and only then shall be allowed to appear.

No particular age criteria as such. Only criteria is based on minimum narks required . No upper cap or lowercap for Age Limit been prescribed .

With this score you should have the chance to get admission in IISER Mohali. However, the cut off for any IISER changes every year depending upon the number of candidates, their choices and overall marks they have scored.
