I can understand that this year despite of scoring such a high score you get such a rank but this shows that how much the competetion is rising and also the cutoff is going to rise so much high .
440 marks are very high but this year nothing much cannot be said at the moment so you have to wait for the counselling results.
It is a very prestigious institute so the cutoff over there is going to be very high this year .So for this collage you will have to go for the Mop up round .

Are you sure it is your All India Rank and not your Category Rank. Because this year an All India Rank of 125719 secured 456 marks. So with a score 452, you cannot have an All India Rank of 54249. Having said that, in 2019, under State Quota, 476 marks secured a seat in Lady Hardinge Medical College under SC Category. That was the cut off. With 452 marks, it is tad difficult for a seat in the Colleges you have mentioned. Having said that, this is based on 2019 data and cut offs change every year. Cut offs change every year depending on various factors such as no.of Candidates appearing for the exam, no.of Candidates qualified, top NEET score and difficulty of the exam. You need to participate in the neet ug counselling process which will start at the end week of this month or first week of November.

Last year cutoff for Delhi domicile was 647 marks foe general category.
With state changing every year you can get admission in MAMC with 580+ neet score.
For Delhi quota MAMC chas 85% seats of total 250 seats. 15% quota is for AIQ ranks and 6 seats for government quota.
General PWD candidate last year score was 223 , OBC 615 and OBC PWD 136 .

To predict your chances in any particular college is tough since the cutoff vary Every year based on various factors and this year's cutoff is comparatively really high, but you can get a certain idea about your admission possibility.
The previous year's rank (2019) for 15% AIQ in Maulana Azad medical college, New Delhi in OBC category was 148 and for the year 2018 was 210. So as you can see the cutoff is really high. This year the cutoff has increased so you can expect more tough competition.

NEET 2020 cut off has not been updated as of now. You will get to know the cut offs once the results are declared.
In order to avail the EWS reservation, you must have the required certificates as a proof. You will be asked to produce the same during the Counselling process. The government has fixed a 10% reservation of seats for General Category students hailing from economically weak section.
You can expect the closing rank of 100-120 for this EWS reservation category.
