Brainware Group of Institutions Kolkata requires the following documents for Chief Minister Relief Fund financial assistance applications:

• A prayer addressed to the Hon’ble Chief Minister for financial assistance, signed by the applicant, including their complete address, mobile number, and optionally, email ID.

• Recommendation to the Hon’ble Chief Minister from the local MLA or MP.

• Family Income Certificate issued by competent authority (Minister-in-Charge, Minister of State, DM, SDO, BDO, Joint BDO, Executive Officer for Municipality areas, or Officer in the rank of Deputy Commissioner and above for Municipal Corporation areas).

• Self-Declaration by the student under their own signature, stating their current course of study with year/semester details and receipt of any Government/Non-Government Scholarship/Stipend. This declaration should be countersigned by the head of the present institution with a stamp/seal.

• Admit Card of the 10th standard examination.

• Mark Sheet of the qualifying examination (Madhyamik or equivalent for assistance at H.S. Level; H.S. or equivalent for assistance at U.G. Level; Graduation for assistance at P.G. Level).

• Rank card of the entrance examination for professional courses such as Diploma Eng., B.E., B.Tech., MBBS, Law, Nursing, Pharmacy.

• Allotment letter for admission in the above-mentioned professional courses.

• First page of Pass Book of the applicant’s active savings Bank Account showing A/c. No., IFSC No., and Account Holder’s Name clearly.

• Admission receipt for the current academic year/semester.
