• HSC Mark Sheets-cum Provisional Certificate (or equivalent Board). If an applicant has not received the mark sheet from the Board in-time, then the download mark sheet from the Internet will be admissible.

• Conduct Certificate from the institution last attended.

• Caste Certificates from revenue office not below the rank of a Tahasildar.

• Schedule Tribes Certificates from revenue office not below the rank of a Tahasildar.

• PH/OH Certificate from CDMO /SDMO/CMO of Capital Hospital (in case of Bhubaneswar)

• Ex-Serviceman Certificate from Zilla Sainik Board.

• Certified true copy of NCC Certificate from the competent authority.

• Certified true copy of the order of transfer & joining in the present station.

• Certified true copy of Migration Certificate for the applicant from other Boards except BSE, Orissa.

• Certified true copy of permanent resident/nativity certificate (for OSA & OLNS candidates)
