According to the previous year cutoff Uttar Pradesh NEET, you can get admission in government medical colleges if you score more than 610 (for General category). It means to get a rank of 15450 or above. Mentioned below are the name of the colleges which are government and offer MBBS with this score and rank:

See, with 261 marks it is very difficult for you to get admission in any government college either Mbbs, Bds, Bams or Bhms.
As you have not mentioned your category I am considering it to be general.
So for general category if you need any government college, you need these marks:-----------------
MBBS - 600+
BDS --580+
Bams-- 550+
Bhms--500+

But , you have qualified the exam for sure according to the cutoff percentile.
So you can get admission in any private medical college in any of the course above according to your interest. All have different procedures and totally different courses.

And there are two defense quota colleges in India one is Afmc pune and the other is acms delhi but both need very high marks if you are from general category.

It's quite difficult to predict that whether you will get  seat or not as  cut off marks/rank for medical colleges keeps changing every year  depending upon various factors like  number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc. As you mention with 249461 rank and you are from general category you have low chance to get seat in government college in MP.

You can get admission in some government colleges but your score is a little bit low. According to the previous year cutoff, you can get admission in krishnadevaraya College of Dental Sciences Bangalore the part of of which was 313 in the previous year. You can also try for KGF College of Dental Sciences and Vydehi Institute of Dental Sciences and Research Centre. Or Maratha mandal Dental College Belgaum, and PM NADA Gowda Memorial Dental College and Hospital Bagalkot can also be your options for admission in NEET BDS course.

Spot round admissions will be done through an online seat allotment process as done for round 1 to round 3.

Spot round 1 and Spot round 2 will be held just like round 1 to round 3. Seat allotment results will be made available on the portal, on the respective student login, as per spot round schedule of the concerned government college

Yes, there  a spot round taken in government colleges for agriculture. There is a online admission counseling and after this the spot round is taken for filling of vacant seats.
