• MBBS Statement of Marks (First year to Final Year) (or) Consolidated Statement of Marks.

• Transfer Certificate and Conduct Certificate issued by the Institution last studied.

• Permanent Medical Registration Certificate issued by the MCI / NMC (or) any State Medical Council.

• Copy of Birth Certificate as proof of Date of Birth
