As of now its quite tough to say because each year the cutoff of colleges changes due to many factors like total students appearing for exam,difficulty level of paper,previous year trends etc but based on previous years trends 121 marks in neet cannot fetch you a seat in medical colleges of India.You need atleast 400+ to fetch a seat in good college.you can refer to the following : NEET College Predictor 2021 - Predict Colleges based on your Rank

• Number of applicants in NEET exam
• NEET question paper difficulty level
• Score of the candidates in NEET 2021 result

NEET 2021 Cut-off goes like : -
• General - 50th percentile
• SC/ST/OBC - 40th percentile
• General-PwD - 45th percentile
