      • Discontinuation Bond (in Prescribed Format)

      • Affidavit ( For Document in Prescribed Format)

      • Medical Fitness Certificate (issued by District Medical Board/Allotted Institute)

      • Self-attested photocopy of Aadhar Card or other ID cards

      Note: Candidates are required to bring at least one set of photocopies of the mentioned documents to Mysore Medical College campus at the time of admission.
