If you have a domicile certificate and prove in UP state then don't worry you are eligible for UP government medical Colleges although you are applying from MP.
But you don't have your domicile prove for UP citizen then you can't apply as a domicile student.

If you belong to gen category then you dont have to take any caste certifcate.but if you belong to BC,SC,ST or other reserved categories you need to take the caste and category certifiactes.below are the list of certifcates you need to take during couselling:
