Since you have not mentioned about any exam or any course, I don't know how much I will be able to help you but here's what the thing is:
There's an All India quota counselling & State Quota counselling, like in NEET. So, if you fill the All India quota counselling form, you will be able to sit for the counselling process of the entire country just with a single form. But if you want to sit for the 85% state quota counselling then you have to fill up the counselling form of the respective state you want to take admission in. For every state, there's different forms for state counselling. So, one application won't suffice.

The application window for KEA has not been opened yet. They will release the admission forms online after NEET 2021 probably as done by them last year.

As we go through previous year's (2019) cut-off then it shows that in 2019 the lowest cut-off marks was 528 for St John Medical College, Bangalore under 85% state quota for General category and we saw an increase of around 25-30 marks in institutional cut-offs in 2020.
So if you are targeting for NEET 2021 then your aim should not be less than 570 out of 720 marks.

According to your query you want to take admission in St John's College , Bangalore . Even though you're a resident of west bengal you can surely take admission in Bangalore without any issue. You'll be required to be eligible for all the rules and regulations of the college and meet the marks criteria. You'll only need an additional document , i.e. Migration certificate from your school. Yes of course you can apply for B-category/Management quota seat irrespective of your marks because the seats are allotted on the basis of merit and also note that if you talking about state quota counselling of AP then you have to follow the schedule which has been released by Dr. NTR University of Health Sciences, Vijayawada for different rank groups.
On such marks you have good chances of getting some medical colleges in Andhra Pradesh under state quota according to previous year's cut-off. As we go through previous year's cut-off then it shows that you can get following colleges :-

for getting admission in st johns medical college bangalore see the previous year cutoff-
General – PH required 45 percentile and marks range is between 467 – 213 .
Unreserved (UR) required 50 percentile and marks range is between 686 – 469 .
Schedule Caste (SC ) required 40 percentile and marks range is between 469 – 353
Schedule TRIBE (ST) required 40 percentile and marks range is between 467 – 304
Other Backward Classes (OBC) required 40 percentile and marks range is between 469 – 466 .
General – PH required 45 percentile and marks range is between 467 – 213 .
As you didn't mention your category but you can see the expected marks which need to qualify.

Maybe should again call the KEA help line center and follow up the details. So that they can be updated about your problem.
I started my Journey in St John's medical college as a MPT Neuro PG since 2023 . In St Johns , Neuro PT dept go hand in hand with PMR dept . We rehabilitate patients from neurology , neuro surgery and PMR department . We get to treat patients with different varieties of neurological disorders . We have a separate Neuro Rehab room for OPD patients. Patient exposure wise St Johns is the best college in Bangalore for MPT neuro program , but when it comes to resources , johns have to improve as Neuro- rehab always prioritise evidence based practice and advancement in technologies to ease the work load of the therapist which results in betterment of patients .
