Hello Jitendra ,

It is difficult to get admission with this marks in any NITs or SLIET Punjab. This year cutoff was approx 2 marks less than JAM 2017. Even after this reduction, You can't get admission NITs.

Cutoff for Any good NIT is above 25 Marks in Open category

no , there is no extra form needed to be filled after mains to get admission. as only first the form is filled up adn submited. you dont need to fill it agin and again
