As you've not mentioned your category, considering you as a general category student it's expected that you need to score above 615 marks in neet UG to get admission in PMCH Patna.
As per previous year records the cut off was 617

See when you are choosing any college for MBBS course then you should choose the one which will give you better clinical exposure and is active in case load generation as well as have well experienced faculties.
As per these parameter I would say JNMC Raipur will be better for MBBS than PMCH Patna as it will give you better exposure to clinical fields such as General Medicine, Pediatrics, Oncology are very strong in this college. So if you go for it then you can get better experience in these fields form JNMC rather than PMCH. And this are the rising fields in medical science which merits research and improvements in clinical methods.

I am assuming you from general category
It's difficult to make prediction but when we go through previous year trends then
In neet 2019 , 617 marks for general category was the cut off marks/ lowest marks that got an mbbs seat in Patna Medical College, Patna under bihar state quota

But yes,
cut off marks / rank for medical colleges keeps changing every year depending upon various factors like
number of students that appeared in the neet examination,
kind of marks obtained by students in neet ,
toughness level of the neet examination,
number of seats available etc.
So , in your Neet 2021 Examination to be on the safer side, so you should target at least 30 marks more in comparison of neet 2019 i. e , you should target at least around 650 marks .
Now that,
limited time is left for neet 2021 , so you should be focusing on your preparation,
practising chapterwise questions
then revising the chapters,
solve at least previous 10 year neet question papers as concept behind the question often gets repeated in the examination so it will be of great help in your preparation ,

Yes, you can pursue MD in the USA after completion of MBBS in India. For that you have to appear for United States Medical Licensing Examination (USMLE).
There are 3 Steps in this exam which you have to clear and that cumulative mark will be considered for giving you the required stream in MD. The Exam consists of both theoretical and practical approach. The Step I of USMLE can be attended from India. But the next USMLE II CS and the Third Step must be attended in the USA!

You will not be paid any stipend when you will do your externship in PMCH.
As per the rules specified by the Medical Council of India (MCI) and most medical colleges, it is possible to study in one medical college and apply for Internship in another but it is a long and complicated process. If you have completed your MBBS from Rama Medical College Kanpur, if you complete your internship there itself as you are familiar with the faculty, system and also entitled for a monthly stipend as per the rules. But if you wish to pursue internship from Patna Medical College and Hospital. This will require you to obtain No Objection Certificates from Rama Medical College, Patna Medical College and the MCI. You will of course have to fulfill the 'externship' eligibility criteria and requirements of PMCH and make it to the merit list to be selected. You may also have to pay a fee to PMCH for your training as specified and will not be paid any stipend when you do externship there.

Patna Medical College and Hospital was established in 1925 and originally known as Prince of Wales Medical College, is a medical college located in Patna. BSc in Medical Imaging Technology program is suitable for students seeking to expand their horizons and pursue a career field related to medical research & development or technological research in companies that lead to a breakthrough in diagnostic imaging. It offers internships for medical students but I am not sure about the BSc students.
