Yes there is B.E. Information Science and Engineering branch in Reva institute of technology and management. Total no of intake is 120 for B.E. level.
The eligibility criteria is --- Student should have passed 2nd PUC /12th standard or equivalent examination with Physics and Mathematics along with Chemistry / Bio-Technology / Computer Science / Electronics / Biology as optional subjects with English as one of the languages of study and obtained a minimum of 45% of marks in aggregate in the optional subjects. In case of candidates belonging to Karnataka Scheduled Caste, Scheduled Tribe and other Backward Classes (Category I, 2A, 2B, 3A and 3B), the minimum marks for the purpose of eligibility will be 40% of marks in aggregate in the optional subjects in the qualifying examination.

No there is no need for application form chalan you need only your admit card and aadhaar card as main documents

Well, your rank is good and you can get ece branch in round 1 counselling of comedk. As per previous years data, closing rank for ece branch have gone upto 30000 or more in many colleges.
